PRN Field Supervisor

Department: Client Services
Location: Philadelphia, PA

**Job Summary:**
The PRN Field Supervisor is responsible for planning, coordinating, and documenting non-skilled supervisory visits for clients in Philadelphia and surrounding counties, ensuring compliance with laws, regulations, and company policies. They directly supervise personal care assistants, conduct field evaluations, and provide mentorship and support to field staff.

**Responsibilities:**
1. Assess new client needs and develop initial care plans upon admission.
2. Confirm clients' preferred care schedules and ensure quality care delivery.
3. Conduct client reassessments and communicate any changes to the care plan.
4. Perform direct observation of PCAs for performance evaluations and monitoring of newly hired staff.
5. Assist in closing care gaps and conduct home visits as needed.
6. Communicate client issues, concerns, and complaints to appropriate staff.
7. Maintain positive client relationships and demonstrate excellent customer service.
8. Manage daily schedule and complete visits in a timely manner.
9. Participate in meetings, in-services, and team meetings as required.
10. Assist with special projects and other duties as assigned.

- Strong interpersonal, written, and verbal communication skills.
- Ability to work collaboratively in a team environment.

**Education and Experience:**
- High School Diploma required.
- CNA or HHA certification preferred.
- Associate's degree or equivalent industry experience preferred.
- At least one year of previous healthcare experience.
- Proficiency with Microsoft Office Suite or related software.
- Must have access to a reliable automobile, valid driver's license, and insurance.

**Physical Requirements:**
- Ability to lift up to 50 pounds.
- Mobility to move within offices and client homes, including stairs.
- Effective verbal communication abilities.

**Travel Requirements:**
- Regular travel in the Philadelphia area and surrounding counties.
- Travel required in various weather conditions.
